Outreach case management focused on sustaining housing and preventing homelessness
The ReStart Supportive Housing Program supports people aged 25 and over who are currently in social housing but experiencing challenges that may place their tenancy at risk or who need support to settle into new social housing. We offer up to 6 months of flexible, outreach-based support to help people stabilise their housing and build a stronger foundation for the future. We can work with people who are housed or moving into housing 10kms from St Kilda.
Please note: We are unable to work with people who have been issued a Notice to Vacate or Possession Order on their tenancy.
Support That Adapts to Life’s Challenges
Our case managers provide flexible, short-term support tailored to each person’s needs and circumstances. While we may not be able to continue support if a tenancy ends, our goal is always to help people make their house a home and feel connected within their community. We aim to link participants with on-going and specialised supports where needed. Working alongside participants to strengthen their housing stability and lay the groundwork for long-term wellbeing.

ReStart is part of Sacred Heart Mission’s Continuum of Care, a trauma-informed and person-centred approach to ending homelessness. Guided by Housing First principles, we use creative, proactive engagement to support long-term outcomes.
