Case Management

Our case management team specialises in providing tailored, appropriate assistance with housing, mental health, legal, medical, social and life skills.

Whether they’re working in the Resource Room or in the Dining Hall, support workers regularly refer people who need more intensive, one-to-one support to the Mission’s case management team.

Responding to over 400 case management episodes a year, including around 10 per cent with people from a range of culturally and linguistically diverse backgrounds, the team specialises in providing  tailored, appropriate assistance with housing, mental health, legal, medical, social and life skills.

The team works with people to identify their goals and support them to make achievable steps toward independence. Assistance is also provided to help people attend appointments, court appearances and obtain referrals to other professional services. Partnerships have been established with Alfred Hospital Psychiatry and Windana Drug and Alcohol Recovery to facilitate access to support and treatment services, and Uniting Care Prahran Mission for mental health support.

With private rental housing expensive and in short supply, it is almost impossible for some of our more vulnerable clients to access decent quality and safe accommodation. As part of our case management services, and with the invaluable support of St Mary’s Parish in East St Kilda, we are able to support vulnerable people to secure accommodation in local apartments. Through the stability a home they have been able to pursue part-time work, counselling and make the most of a range of other supports, such as building up independent living skills, that help them address the underlying causes of their homelessness and create a new positive direction in their lives.
