Our two supportive housing teams consist of skilled case managers who support people aged 25 years and older to obtain and sustain long term housing.

Outreach case management, with an aim to end homelessness

The Supportive Housing Program has two teams (Women’s Supportive Housing and General Supportive Housing) of outreach case managers who work with people 25 years and older, to obtain and sustain long term housing.

The Supportive Housing Program supports people into long term housing and stick around to support people to make a house a home.

Through floating case management support, the Supportive Housing Program can remain involved with someone through different experiences, and can continue to provide support even if a person’s tenancy ends. A case management period in this program can commonly be 6, 12 or 18 months, with capacity for up to 2 years. Having capacity for flexible support, this program is able to work with people to set down roots in their community and build a steady foundation to continue with other goals in their life.

The Women’s Supportive Housing Team is available to women (cis and trans) and non-binary folks who feel this team is for them. This team utilises an intersectional feminist lens in their approach to support and advocacy for those accessing this service.

A journey of learning to create a program of connection and creativity

The Supportive Housing Program is an integral part of Sacred Heart Mission’s Continuum of Care – an approach to connecting Sacred Heart Mission programs, prioritizing Trauma Informed Care and ending the experience of homelessness.

With a focus on support that is inspired by Housing First principles, the Supportive Housing Program uses creativity and assertive engagement approaches within a sustaining tenancies lens.
