As an endorsed specialist service provider to people in the community living with deep, persistent disadvantage, social exclusion, and people who are experiencing or are at risk of homelessness, Sacred Heart Local provides expert in home supports. Our focus is on providing skilled Care Partners, Social Workers and Support Workers with the time to get involved where family or social networks may not be available. We join with our participants to develop and deliver holistic supports and services, embedded in a trauma informed approach. Our unique service enables us to provide you with social worker supports to assist when you need them.

Our aged care services
We know that many people who are ageing experience loneliness and social exclusion, especially if they do not have strong support networks.
Sacred Heart Local offers support through Case Management, Social Work and Service Co-ordination to assist to co-ordinate support that is best suited to the participants needs. Our staff are reliable, trustworthy and form strong connections to clients and persevere where needed to support clients to achieve their goals.
Sacred Heart Local provides aged care services under the Commonwealth Home Support Programme (CHSP), or Support at Home Program (SAH) for people aged:
- 65 years or older (50 years or older for Aboriginal or Torres Strait Islander people), or
- 50 years or older (45 years or older for Aboriginal and Torres Strait Islander people) and on a low income, homeless, or at risk of being homeless.
Eligibility
- To access federally funded aged care services (both CHSP and SAH) you must first be assessed through My Aged Care using the Single Assessment System (SAS). This assessment will identify the type and level of care you need. Contact My Aged Care on 1800 200 422 or at the My Aged Care website.
- If you need translation services to complete your application call the Translating and Interpreting Service on 131 450, tell the operator which language you speak and ask the interpreter to call My Aged Care on 1800 200 422.
- First Nations people can call My Aged Care on 1800 200 422 and ask for Interpreter Connect. The Elder Care Support Program can also assist connecting First Nations people with Aged Care Supports.
- People who have a hearing or speech impairment can contact My Aged Care through the National Relay Service. Select your preferred NRS access point and provide the My Aged Care Number – 1800 200 422.

Our Programs
Commonwealth Home Support Programme (CHSP)
Our Commonwealth Home Support Program provides support to people living in Southern and Northern Metropolitan regions who require assistance with:
- Personal Care (North only)
- Domestic Assistance
- Hoarding and Squalor Support (South only)
- Social Work
CHSP is considered to be entry level services to meet the needs of people who have low level care needs. Our Social Worker Program can work intensively with our participants to support linkage into long term Aged Care Supports, including supporting people with their Aged Care Assessment.

Support at Home
The Support at Home program is the new way the Australian Government funds in-home aged care, replacing the former Home Care Packages program.
How It Works
To access federally funded aged care services under Support at Home, you must first be assessed through My Aged Care using the Single Assessment System (SAS).
After your assessment, you will receive a Notice of Decision. This letter includes:
- Your approved classification level (1–8) based on your care needs.
- Your funding allocation and what services you can access.
- Any short-term supports approved (e.g., Restorative Care, End-of-Life Care, Assistive Technology & Home Modifications).
Support at Home offers eight levels of ongoing support, from minimal help to very high care needs. These levels determine your funding and the types of services you can access.
- Minimal Support – Occasional help with light household tasks.
- Light Personal Care – Basic assistance with showering, dressing, and social support.
- Moderate Assistance – Regular visits for hygiene, mobility, and meal preparation.
- Low-to-Moderate Support – Includes care coordination and chronic condition support.
- Moderate Support – Higher-level personal care, nursing, and allied health.
- Moderate-to-High Support – Daily support with medication and complex care planning.
- High Support – Intensive personal care and frequent nursing visits.
- Very High Support – Multiple daily visits, complex clinical care, and carer support. Your Care Partner
Once approved, your Care Partner will:
- Work with you to agree on your goals and priorities.
- Help you understand your Notice of Decision and what services you can access.
- Coordinate services within your approved budget.
- Support you to request a reassessment through My Aged Care if your needs change
Examples of Services
- Personal care (showering, dressing)
- Light domestic cleaning
- Meal preparation and shopping
- Transport to appointments
- Nursing and allied health (such as physiotherapy, occupational therapy, dietician)
- Social and community participation
- Light gardening and home maintenance
- Social Work
- Assistive technology
Understanding the Aged Care Statement of Rights
The Aged Care Statement of Rights sets out what you can expect when receiving Australian Government-funded aged care services under the Aged Care Act 2024.
It ensures that everyone receiving aged care services has the right to:
- Be treated with dignity and respect
- Receive safe, high-quality care and services
- Make choices about your care and have your preferences respected
- Have your privacy protected
- Be free from abuse and neglect
- Access information about your care in a way you understand
