Our Sacred Heart Local services launched in March 2017, have expanded our in-home support services throughout the northern regions, and outer south in Casey and Greater Dandenong. The program provides a package of in-home support services for vulnerable and socially isolated individuals living in the community, whether through Home care packages, the Commonwealth Home Support program or NDIS.

Sacred Heart Local provides NDIS support

Sacred Heart Local was developed to extend our traditional Home Care service, which was available to clients over 65 years of age, to include support to adults aged under 65 years who are eligible for NDIS support packages. This includes clients who were eligible for Home Care Packages, but are now ineligible because of the introduction of My Aged Care assessor portal and a tightening of the age criteria.

As a specialist provider of Home Care Packages (HCP) and now National Disability Insurance Scheme (NDIS) packages for adults, Sacred Heart Local will provide a range of support to eligible clients to maintain their housing and independence in the community.

Sacred Heart Local packages will make life easier for disadvantaged community members by providing:

  • Case management or support coordination
  • Meals delivery
  • Assistance with shopping, meal preparation, and self-care
  • Cleaning
  • Assistance with daily living
  • Transport to appointments and activities
  • Social and community participation.

We work with people who experience a range of complex issues such as:

  • Mental health conditions (psychosocial disability)
  • Living alone with no family network or limited informal supports
  • Substance abuse
  • Histories of homelessness
  • Trauma
  • Living alone with no family network or limited informal supports
  • Long-term unemployment
  • Hoarding
  • Chronic health conditions
  • Physical and intellectual disability including Acquired Brain Injury (ABI).
