Our aged care services

We know that many people who are ageing experience loneliness and social exclusion, especially if they do not have strong support networks.

Each Sacred Heart Local client has their own case manager to coordinate the support that is best suited to their situation and needs. Our staff are reliable, trustworthy and form strong connections to clients and persevere where needed to support clients to achieve their goals.

Sacred Heart Local provides aged care services under the Commonwealth Home Support Programme (CHSP), or Home Care Package (HCP) for people aged:

  • 65 years or older (50 years or older for Aboriginal or Torres Strait Islander people), or
  • 50 years or older (45 years or older for Aboriginal and Torres Strait Islander people) and on a low income, homeless, or at risk of being homeless.

Eligibility

  • To access federally funded aged care services (both CHSP and HCP) you must first be assessed by the Aged Care Assessment Team (ACAT). This assessment will identify the type and level of care you need. Contact My Aged Care on 1800 200 422 or at the My Aged Care website.
  • If you need translation services to complete your application call the Translating and Interpreting Service on 131 450, tell the operator which language you speak and ask the interpreter to call My Aged Care on 1800 200 422.
  • People who have a hearing or speech impairment can contact My Aged Care through the National Relay Service. Select your preferred NRS access point and provide the My Aged Care Number – 1800 200 422.

Commonwealth Home Support Programme (CHSP)

The Commonwealth Home Support Programme (CHSP) is a government-funded program, designed to help older people remain independent and stay in their homes and communities for longer. It is best suited to people who want to stay in their own home and need a little help with daily tasks to keep living independently.

Through the CHSP, you may be eligible to receive domestic assistance, social support, help with meal preparation and transport to appointments.

Home Care Packages (HCP)

Home Care Packages (HCP) are for people who require more support services on an ongoing basis. A range of different services can be provided across three categories: to keep you well and independent, to keep you safe in your home and to keep you connected to your community.

HCPs are provided under four different levels, with different funding amounts, covering basic support needs through to high care needs. As everyone’s care needs are different, there are four levels of Home Care Packages with different funding amounts. These package levels cover basic support needs through to high care needs.

Understanding the Aged Care Charter of Rights

From 1 July 2019, a new Charter of Aged Care Rights will provide the same rights to all consumers, regardless of the type of Australian Government funded aged care and services they receive. The Charter of Aged Care Rights is made under the Aged Care Act 1997.

It says that everyone receiving Australian Government funded aged care services has the right to be properly looked after, treated well and given high quality care and services.
